Is it drawn or is it a render?

It looks like a render with the detailed skin textures, but it also shows pencil markings.

It looks like it's drawn, though I can't tell how. It's obviously not a render because the faces and weapons seem to be fairly inaccurate to the models found in game. As for the pencil markings, those seem to be there purely for stylistic reasons.

Keep in mind that if it is a drawing, it's not a pencil-crayon type of setup, to even call it a drawing might be an understatement. This is professional level multimedia art. And it certainly made it way into Adobe Photoshop at one point or other.

If you haven't see in before, go to DeviantArt.com and look up some photo realistic drawings. You'll be amazed with what some people do in their spare time.

I just cropped it, keeping the width the same - there's plenty of space at the top and bottom that can be cut off. It makes a great background or screen saver.

From the looks of it, I'd say it was a painting. Probably an early render for the box art based on the people they used to model the game characters. The artist certainly is talented - the faces look very realistic.

I hope Left 4 Dead is eventually made into a movie - that's one zombie movie I'd definitely like to see.

On the sniper rifle issue, I must admit I've never even tried it, so I don't know. I've tried every weapon except that one.
